GREAT-ix submission form

This has been produced using Formidable Forms.

This stores a record of all form submissions on this website. Your data can be deleted by the site administrator at your request and you may also request a report of saved data related to your name or email address. Contact us

Some data may be shared with other services including E2PDF (this produces the PDF documents that we use for the thank you letters and cards).

On submission of your form, the information entered is delivered to an NHS email address that is accessed by the local GREAT-ix Team to manage the scheme. A confirmation email with a summary of the information entered is also sent to the form submitter at the email address entered on the form.

The information submitted via this site will only be used to manage the GREAT-ix scheme. If you have given us permission to publish your comments then we may do so to help promote GREAT-ix and encourage excellence.

Contact forms

Our contact form collects your name and email address, alongside the message you have sent us.  This information is only used to respond to the messages/queries submitted and deleted from the email account once the query has been dealt with.
